Achieving a watertight dural closure is always important. It is even more important in the case of skull-base procedures where transsphenoidal, extended transsphenoidal, and translabrynthine approaches are used. When the arachnoid is opened the requirement to achieve closure is essential. Failure to achieve closure may lead to meningitis and other serious sequellae.
Skull base surgery has traditionally involved large transcranial, transsphenoidal, or transfacial approaches with a relatively high morbidity.
